Client records are handled in line with laws on data protection and confidentiality.

You have the right to be given a copy of your consent form and treatment record/s.

You have the right to object to sharing records with other health services.

You have the right to have any mistakes corrected and to complain to the Information Commissioner’s Office.

Information about you/your animal, health and treatment are held so the best possible treatment plan is given.  This personal information can be held in a variety of formats including paper and electronic.

Personal information is collected directly from you.

I use electronic forms on my website for people to make enquiries or to provide feedback.

Records are used to reflect on your treatment/s and to ensure I have accurate, up to date information.

The personal information may also be used to remind you about appointments and any correspondence.

Where possible, I will look to minimise and anonymise your personal information to protect client confidentiality, unless there is a legal basis to act otherwise.

I will not disclose any personal/health information to third parties without your explicit consent, unless there are exceptional circumstances, such as when the health and safety of others is at risk or where the law requires it.

I have a duty to:

• keep records about you confidential and secure

• provide information in a format accessible to you
